MDS Coordinator - Clinical Case Manager ? RN Job Description A client of Personified is currently seeking a Clinical Case Manager/MDS Coordinator to join their team in Idaho Falls, ID. The MDS Coordinator is responsible for the development, completion and transmission of the resident assessment in accordance with the regulatory requirements; reviews and manages reimbursement in compliance with policies and procedures; responsible to case manage residents covered by both Medicare and Managed Care insurance programs. Job Responsibilities ? Comply with applicable policies and procedures including but not limited those within the Compliance Process, Code of Conduct, Federal False Claims and HIPAA ? Participate in required orientation and training programs ? Support a positive and professional image through leadership activities ? Coordinate the completion of the resident assessment (MDS), Triggers and RAPs in accordance with current rules, regulations and guidelines ? Ensure completion and submission of MDS data within regulatory timeframes ? Coordinate the development of comprehensive assessments and care plans of each resident ? Conduct/coordinate facility daily PPS meetings and IDT Process and Claim review ? Assist with education and overall plan of care ? Cooperate with monitoring and audit functions and investigations ? Participate in the review Quality Indicator report and Resident Specific Profile ? Participate in educational programs to maintain knowledge current with RAI/PPS regulations ? Maintain and implement department policies and procedures; ensure staff compliance ? Ensure all OSHA and safety standards are followed ? Adhere to location protocols and department/company policy and procedures ? Adhere to established HIPAA confidentiality standards ? Perform all other tasks or duties as assigned Job Requirements ? Registered Nurse required ? Registered Nurse licensed in ID required (or compact state) ? Minimum 1 year of MDS experience (residential long term care) ? Proficient in RAI/PPS processes Benefits ?
